• Name of the Deceased
• Your name, address, and phone number
• Three possible dates per name
• A check for $20 per Mass
• Limit two names per mass. If there are multiple family members, write in “Deceased members of [family name] family”

We will not process your request if you do not send in a check with your request. We will send you a receipt with the dates for your scheduled Masses. We will give you a call if there are any issues.
